Connecting all of Colorado to quality, coordinated care.

Over 70 percent of the counties in our state are classified as rural or frontier. Where you live shouldn’t preclude you from access to quality local care. For 30 years, Centura Health has effectively partnered with local physicians and hospitals in small towns across Colorado to deliver close-to-home care. Now, Outreach Health closes the gap further, supporting local providers with education and resources. We are connecting the best of the Centura Health network to everyone in Colorado, helping to provide the right care, at the right time, in the right place.

Flight For Life® Colorado

Supported by an anonymous grant, Flight For Life® created “Flight For Life® on Tour,” a series of day-long conferences with topics of interest to rural providers including EMS and first responders in Steamboat Springs, Yuma, Trinidad, Summit County and Divide.

FAST FACTS

The following are a few facts that we would like to call out from FY2010.

Nearly one in three adults living in rural America is in poor to fair health, and nearly half have at least one major chronic illness.

Approximately 37 percent of rural residents do not have access to a primary care physician.

Only 10 percent of physicians practice in rural America, despite the fact that nearly a quarter of our population lives there.

Nearly 70 percent of people living in rural areas have limited or no access to public transportation.

We have partnered with UnitedHealthcare to provide high-quality health care to patients in outlying communities through two-way technology, connecting patients to specialists hundreds of miles away.

As part of this partnership we launched a pilot program in four communities, bringing health care close to home:

  • Buena Vista Family Practice Clinic
  • High Plains Community Health Center – Lamar
  • Rio Grande Hospital – Del Norte
  • St. Vincent Hospital – Leadville
